How to run index.php

I hate to ask a dumb question but is index.php meant to be run as a command line? That is
php index.php
or php -r index.php

Or do I set up nginx to run php. The suggested conf file is appropriate since i have nothing at /var/www/ etc.

I am trying to install the program on opensuse on a notebook, not a R Pi.

just install set up nginx to run php
have nothing at /var/www/ because that directory isnt the same in different pc
