`list_integrations` tool should list integrations in the scope of the target app host.

This is causing confusion for the LLM when it tries to install the package for the preview but it might be running on a PR build for example. Also the `get_integration_docs` command may need to be enhanced to extract the README.md file content from the package on local disk instead of from nuget.org.
