Duplication of ideas in GPT-5.1-Codex-Max Prompting Guide

Duplicated overlapping directives in the starter prompt.

[Problem]
There are several places where the same ideas appear with different wording. mainyl in terms of "bias to action", "don't stop early", "deliver working code".

[Solution]
Collapse these directives under "Executive philosophy" section will make the rest feel cleaner and less repetitive. Also, we need to add a harder line between rules that are absolute v/s preferences but maybe that deserves it's own issue?
